What are the costs involved?
SAH funds are allocated dependent on assessment and needs.
SWCC Services
Once you’ve been assessed and approved to receive Support at Home, you’ll be assigned a Support at Home classification. This classification determines the level of government funding available to help meet your assessed care needs.
Government funding
The amount the government contributes depends on:
your ongoing and/or short-term classification
any supplements you’re eligible for
the services, equipment, and products you receive
your personal contribution toward your care
When you pay your contribution, the government covers the remaining cost of that service directly to your provider. The government also pays the full cost of clinical supports.
Your SWCC budget
You’ll be allocated a quarterly budget, covering a three-month period. The amount of funding you receive from the government is based on your ongoing classification.
Any unspent funds from the previous quarter will be added to your new budget — up to a maximum of $1,000 or 10% of your quarterly allocation (whichever is higher).
Working with SWCC
SWCC will collaborate with you to identify the types of care and services that best meet your needs.
You’ll be able to choose from a list of approved services identified during your assessment. These will be outlined in your individualised care plan, along with a detailed budget.
To help you stay informed, you’ll receive a monthly statement summarising your budget, spending, and any remaining funds. Each level of home care package provides a different subsidy amount. This amount is paid to an approved home care provider that you have selected, in this case South West Community Care. The subsidy contributes to the total cost of your service and care delivery. It is also expected that you will contribute to the cost of your care where your personal circumstances allow.
If you are not sure of your subsidy rate, please ask your Care Partner- call (08) 9782 4000.
