The Commonwealth Home Support Program (CHSP) provides government-subsidised support for people aged 65 and over who need a little extra help to stay living at home.
Through CHSP, South West Community Care offers affordable, local services that focus on your independence, safety, wellbeing, and staying connected to your community.
At SWCC, we work with you – not for you. Our approach is about helping you feel confident and supported at home, while maintaining the routines and independence that matter to you.

How does CHSP work?
CHSP is designed for older people who can manage most day-to-day activities but would benefit from some support.
You may be eligible if you are:
- 65 years or older
- an Aboriginal or Torres Strait Islander person aged 50 or older
- 50 or older and experiencing, or at risk of, homelessness
If you are approved, you’ll receive a letter outlining the services you’re eligible for, including any short-term support based on your needs. You can then choose a provider you trust – and we’re proud to be one of the local organisations offering CHSP services in the South West.

How much will it cost?
CHSP is subsidised by the Australian Government. If you can afford to, you may be asked to contribute to the cost of your services.
Fees vary depending on the type of support and the provider. Everyday services such as domestic assistance or meals may cost only a small amount, while more complex supports (e.g., home modifications) may cost more. Your provider will explain the exact costs before services begin.
How long will it take to receive support?
You will first need an assessment through My Aged Care. Timeframes vary depending on the services you need and the availability of local providers. Once approved, we will work with you to start supports as soon as possible.
