Join our weekly outings which are varied monthly.
We visit nurseries, places of historical interest, shopping malls, local towns in the South West, museums, dams, beaches as well as trips to Perth to the casino and to see the flowers at Araluen, theatre shows and so much more.
Enjoy morning tea and /or lunch with friends while you explore and get out and about.
Gardening
At South West Community Care, our Home Maintenance crews can come to your home and help with your outdoor areas and basic safety and maintenance tasks inside.

Nursing
Our Clinical Manager will conduct an initial assessment with all new HCP consumers.
This assessment includes:

- Medications
- Pain levels
- Skin integrity and wounds
- Functional status
- Sensory
- Oral care
- Nutrition and hydration
- Personal care
- Bladder and bowel function
- Respiratory issues
- Sleep pattern
- Diabetes—if applicable
- Braden Scale assessment
- Psychogeriatric assessment
Our nurses can provide the following services to you in the comfort of your own home:
- Health Reviews
- Health Assessments
- Personal care
- Wound Care in your own home– care of ulcers, skin tears, pressure injuries, abscesses or post-surgical wound care.
- Medication support and administration
- Stay on Your Feet Education
- Catheter Care
- Stoma Care
- Continence management and referral to specialist if required
- Catheter care, support and education
- Education, support and monitoring of various chronic conditions e.g. Diabetes
- Referral to Allied Health professionals e.g. Occupational Therapist, Physiotherapist, Podiatrist, Dietitian, Speech Pathologist.

Allied Health Services
We offer allied health services, initial and therapy assessments which will assist us in providing services to meet your needs.
Respite for Carers
Sometimes, your carer might need some time out.
You can enjoy some time with your Care Worker, either at home or out for a few hours to give your spouse or carer a break.
Transport
SWCC transport is provided for:
Any medical appointments – these requests take priority
A social centre activity day – get transport to the social centre activity days
An outing with SWCC to join others from our community
A trip to the shops– these appointments may have to be flexible times and dates depending on demand
Other appointments — will depend on availability of vehicles and drivers.
Please note that this service is for your convenience . We cannot include friends or family members in your trips, unless they have been approved by management and form part of your caring support. All transport will be invoiced– no cash payments required. Transport is charged based on distance travelled.
Transport with volunteer drivers
We have a team of volunteers who willingly give of their time to support transport needs. If you have to travel somewhere between 8 am and 4pm, our volunteers can assist you. They collect a vehicle from one of our offices, come to your home and take you to your requested destination. We also transport you home after your appointment.
Transport with care workers
Our care workers can be scheduled to assist you with transport requirements throughout the day. We have care workers who are able to assist if your medical appointment requires an earlier start than 8am or a return after 4pm.
PLEASE NOTE: a minimum of 24 hours notice to be given to book or cancel transport bookings. Weekends are excluded from this notice period.
